Submissions

Are you a Crosspoint member looking to submit your clearances to participate in children’s ministries? If so, you’ve come to the right place. Below you’ll find all the applicable links to get started.

All childcare workers must fill out the Crosspoint Child Workers Application.

In accordance with Pennsylvania law the following background checks must be submitted with the Crosspoint Child Workers Application and renewed every five years:

  • All childcare workers must fill out this application.

  • This check can be performed online, is available in most cases instantaneously, and should be printed out for your personal records. ***Warning: If this background check is submitted online you will only be allowed to view the report once. Please print the report out to submit it with this application. The background check may also be submitted through mail. Printable forms are available here. The current cost for this background check is free.

  • This check can be performed online, is available in most cases instantaneously, and should be printed out for your personal records.   • 4A. Affidavit of Residency -The signed affidavit is available only to those who have lived in the state of Pennsylvania for the last ten years or more. If this does not apply to you, you must obtain an FBI Criminal Background Check (see 4B). Printable affidavits are available here: (Adult Affidavit / Minor Affidavit)

    4B. FBI Criminal Background Check - This check can be performed online and the current cost for this background check is $23.25.

All documents should be submitted to admin@crosspointbaptistchurch.org or hand-delivered to the administrative assistant.
